Vulnerable Elderly Patients

We are committed to providing the best support and care we can to all our patients, but often it is those elderly patients living at home alone who are overlooked because they do not want to bother us. Then they become ill and often go into hospital when, with a little more help and support this could be avoided.

Every member of our team has allocated to them several patients who, with their consent, they contact on a regular basis to make sure they are safe and well and looking after themselves. The patients appreciate this contact and we can look out for times when we feel that further medical and social support is needed. Ultimately this saves hospital admissions, but the reason for our scheme is to look after a growing population of elderly adults who need and deserve our support.

Visiting our practice?

Please continue to wear a face covering and social distance

All of our COVID-19 secure measures remain in place and this will not change when Government guidance changes on 19 July 2021.

Patients are asked to continue to wear a face covering, unless they are exempt, and to follow social distancing when visiting our GP Practice.

This follows the national Infection Prevention Control (IPC) Guidance for all healthcare settings and will help to ensure the safety of our staff and patients at all times.

We are continuing to follow COVID-19 secure measures and they will remain in place until further notice.

Our Walk In service has changed due to COVID and we are unable to offer you a sit and wait service.  If you would like to see a Clinician, please contact 01480 219317 and we will arrange for GP or Advanced Nurse Practitioner to call you back, we currently do have an increase in demand and therefore appointments are limited, so please check to see if you can get an appointment with your own GP first.
